Know Your Numbers Health Screenings and Flu Clinics – Sign Up Today and Don’t Miss Out!

 

As part of the Wellness Your Way incentive program, full-time, benefits-eligible employees are invited to attend an on-site health screening, which also will count toward their 2019 Tier One Wellness Your Way requirements. You do not have to participate in the incentive program to take advantage of the screening.

 

In addition to the Know Your Numbers health screening, we have arranged flu vaccine clinics to align with all upcoming health screening dates through December 2018 for all campuses. Follow the How to Schedule instructions below to register for your appointment. Registration is required.

We only will be hosting one health screening and flu vaccine clinic per Regional Campus. Employees are welcome to attend any clinic that best fits their schedule. But don’t delay, register today!

 

How to Schedule Your Appointment

  • Once you are logged in, select My Appointments, then Worksite Screening. Choose the date and location that best meets your schedule. Please keep in mind that you will need to fast for 8 to 10 hours prior to your health screening and blood draw via a finger prick.
  • To complete your online health risk assessment while logged into Be Well, select My Wellness and then select Health Risk Assessment.
     
  • Results of your screening remain confidential at all times and will be provided to you by Be Well at the time of screening.
